This model is centered around our Relatives and is designed to be comprehensive, accessible, and continuous. Our goal is to build strong, supportive relationships between the care team and our Relatives and their families. In this patient-centered approach, we encourage and empower our Relatives to actively participate in decision-making alongside their care team.
