Established in 2014, the Great Plains Good Health and Wellness (GPGHW) program, has used evidence-informed, community rooted health promotion and disease prevention initiatives to help Great Plains area tribes combat high rates of obesity, commercial tobacco use, diabetes, heart disease, and stroke. In September 2024, the GPGHW Program was awarded with the third cycle of GHWIC. Focusing on community and social connectedness leading to healthy lifestyle change that promotes health and wellness.
GPGHW provides subawards each year to Tribes/Tribal entities in the Great Plains Area to:
- Provide a foundation for chronic disease prevention by implementing community-chosen, traditional AI/AN practices that build resilience and strengthen connections to family, and community.
- Establish and enhance systems that connect community members to community, clinical, and social services and programs to prevent, manage, and control chronic diseases and their risk factors.
- Promote the implementation of multi-disciplinary team-based care to prevent, manage, and control chronic diseases and to connect patients to community and social service providers to address health-related social and economic needs.

Criteria For Program:
- American Indians of any age living in the Great Plains area
